Customize Your Signup Form

Collect demographic and personal information about your community to establish your community database.

This task is for Site Administrators.

Participants register on your site via the Signup Form. The form collects demographic information, which you can then apply to your reporting analysis. It also restricts access to tools with registered participation and protected projects.

You can customize your Signup Form using a variety of question types to collect the demographic and personal information required to establish your Participant database. Please note that your Signup Form is unavailable on your live site if your site lock is on.

Participants can manage and update the information they provide when they create their account in the Personal Info section of their profile.

As the Signup Form controls what demographic information you collect, you should plan out what questions you want to ask before you launch your site. We recommend reading up on the benefits of registration for your EngagementHQ site, as well as the implications of changing your Signup Form.

Add or Edit Questions to Your Signup Form

Before you start adding or editing questions, please note that there are several questions that you can edit but not delete:

  • Login

  • Account

  • Password

  • Terms of Service

You cannot remove these as they are all necessary details for a registered account. To start customizing your Signup Form:

  1. Go to Site Settings > Signup Form from the main menu to see the existing list of questions.

  2. To add a new question, select Add, choose your settings, and then select Create.

  3. You can also select Edit to change the text or settings of an existing question; select Update after making your changes to save. Please note that you cannot change the Question Type of an existing question; you must delete and recreate it instead.

  4. Select Delete to remove a question from your form.

  5. Reorder your questions or question options using the grey border to drag and drop them within your form.

  6. Select Preview to test your form before launching your site.

You can also include reCAPTCHA on your form by going to Site Settings > Privacy and Security > Sign Up Form and toggling Enable Goggle’s “reCAPTCHA” across your site.

Use Participant Demographic Attributes in your Signup Form to assist with accurate demographic analysis in your Dashboard reports. You can attach the Age attribute to the Date question and the Gender and Ethnicity attributes to Radio buttons, Checkboxes, or Dropdown questions. You can apply each attribute to one question at a time. Participants can manage and update the information they provide when they create their account in the Personal Info section of their profile.

There are several settings you can choose when creating a question; please note that some settings vary by question type.

Use this drop-down menu to select your question type. We provide several types of questions, all common to our survey questions.

Demographic attribute

Date, Dropdown, Radio Buttons, Checkboxes

Assign a Participant Demographic Attribute to your question to assist with accurate Dashboard and Participants analysis. You can only assign an attribute to one question at a time.

You can assign the following:

  • Age: to the Date question only

  • Gender: to Radio buttons, Dropdown, or Checkboxes

  • Ethnicity: to Radio buttons, Dropdown, or Checkboxes

Use this field to type your question.

Maximum Length

Single Line, Number

Set a maximum character length for the participants’ answers.

Add option ‘Not in [region]' to Suburb Question

Suburb

Check this box to add “Not in [region]” as an additional answer to this question.

Required

All question types

Check this box to make the question mandatory before users can submit their registration.

Collect only year

Date

Check this box to allow participants to only give their birth year instead of the complete date.

Display options on the same line

Radio buttons, Checkboxes

Check this box if you want all the options for your Radio buttons and Checkboxes to appear on the same line. Please note that this will be responsive to the size of the screen and the length of the options.

Use the text boxes to enter your answer options. By default, there are four text boxes. You can select Add more for additional options.

Add additional text to explain how users can answer the question. This is also a good place to reassure users about how you will use the information they provide and what you will do to protect their privacy.

Use Image Link

Image

Display an image in your Signup Form. Choose to:

  • Link to an image: the Image Link field will appear for you to paste in the image’s URL.

  • Upload an image: select Browse to upload an image from your computer.

Caption

Image

Provide a caption for your image. This will also be the image’s alt text, so it is a required setting.

Show this question only when...

All question types

This setting allows you to set conditional questions on your Signup Form. Conditional questions only show when a previous question was answered in a specific way and can help you collect additional information without making your form too long.

For example, a parent Radio button question could be, “Are you willing to answer a few more questions?” When a participant answers Yes, a series of extra questions appear while selecting No would lead directly to the form submission button. Creating a long Signup Form can become a barrier to participation; you want to ensure your participants can register and contribute as quickly as possible. Because of this, you cannot add additional pages to your form, but you can add a Section if you want to separate a group of questions.

The Section Question type provides text fields for a Section heading and Description, and you can make it a conditional question. Sections help explain how the information provided will be used or for introducing a series of connection questions.
